A unique approach to health and care, the clinically-led multi-disciplinary team treat thousands of frail older people with long term conditions each year across Hull, who have been assessed by their GP as being at risk of hospital admission.
Opening in 2018, the award-winning Jean Bishop Integrated Care Centre was built to support more patients closer to home in the local community.
About the Jean Bishop Integrated Care Centre
Jean Bishop Integrated Care Centre (ICC) opened in 2018 to support elderly, frail patients closer to home in the local community; from 2018-2023 more than 12,000 people have been seen, assessed and supported with individual care plans.
The centre gives elderly people access to both physical and mental health advice from geriatricians and GPs, and nurses specialising in frailty, occupational therapists, physiotherapists, pharmacists, and social care teams.
The centre operates as a hub for rapid assessment, coordinated care and early intervention across for Parkinson’s Disease, dementia, and COPD.
The ICC also has diagnostic services, including ultrasound, X-ray, and phlebotomy. There is an on-site fire station with the team from the Humberside Fire and Rescue Service assessing safety issues for older people. The building is mainly single story for easy accessibility. There is a cafe run by a voluntary group providing a place to relax. There is a sensory garden is part of continuing improvements to the ICCs green space, providing a more welcoming, therapeutic environment for patients.
The benefits are real - frail elderly patients have been diverted away from A and E. The system has seen a drop of 56% in all patients attending A and E as well as an 85.7% drop in regular attenders.

This case study outlines how the team in Hull cut emergency admissions for frail people by 50% by seeing patients at the Jean Bishop Integrated Care Centre, a unique initiative of the City Health Care Partnership (CHCP), a social enterprise working in Hull. Partnership working
The ICC was developed through CHP’s LIFT (Local Improvement Finance Trust) programme. The building is owned by Citycare Hull Ltd. (LIFTCo) of which CHP owns 40%. The land was provided by Hull City Council, and CHP has a 25-year head lease for the building.
Centre of Excellence for Frailty
A new Centre of Excellence for Frailty for Humber and North Yorkshire was launched in July 2024 based on the pioneering work of the Jean Bishop Integrated Care Centre in Hull.

Health Secretary visit August 2022
Steve Barclay, MP, Secretary of State for Health and Social Care, visited the Jean Bishop Integrated Care Centre in Hull in August 2022 to open the memorial garden created in the fundraiser’s honour and to tour the centre, meeting patients, staff, and visitors. He was joined by Jean’s family and friends for the opening ceremony. The garden is now a well-used, calm and relaxing environment for patients, staff, and visitors.
