Community Health Partnerships (CHP) has today published its Year of Impact-Delivering Neighbourhood Health, highlighting a year of significant delivery in supporting the NHS shift towards community-based care.
The publication sets out how CHP has worked with NHS partners, local systems and communities to deliver practical improvements across the NHS LIFT estate, helping to expand capacity, improve access and bring services closer to home.
During 2025/26, CHP delivered its largest capital investment programme to date, investing £17.84 million in estate improvements. This has enabled over 260,000 additional patient appointments each year, alongside expanded diagnostics and increased clinical capacity within communities.
Through national programmes including Neighbourhood Rebuild, PACE (Productivity Acceleration in Community Estates) and Securing the Future, CHP is playing a leading role in transforming how care is delivered, whilst supporting the move to more integrated, neighbourhood-based models of care.
The publication further highlights how targeted interventions from CHP, ranging from large-scale developments to small targeted improvements are delivering practical, and measurable impact for patients and communities
