Neighbourhood Health in Action using existing community-based assets

Community Health Partnerships (CHP) is pleased to publish ‘Neighbourhood Health in Action’, a publication showcasing how the CHP LIFT estate is already delivering neighbourhood care in local communities.

The Government recently announced the first 27 projects that will form the first wave of neighbourhood health centres, supporting the wider ambition to deliver 250 health centres by 2035. 16 of these 27 sites are located within the LIFT estate, further reinforcing existing assets, such as NHS LIFT remains one of the most effective enablers in delivering a neighbourhood health service.

“The Government’s mission to deliver Neighbourhood Health Services, as set out in the 10 Year Health Plan, speaks directly to the purpose of the LIFT estate. Through collaborative working we are supporting the transformation of neighbourhood care, as demonstrated by the many examples highlighted in this publication. To deliver Neighbourhood Health Centres in every community, the 10 Year Health Plan places a clear emphasis on making better use of our existing estate. NHS capital investments show how spaces can be adapted and repurposed to improve productivity and maximise value from community based assets, helping to shift care closer to home. Our work is already contributing to the national ambition for neighbourhood health, and I hope this publication inspires our partners to act on opportunities available through existing estate.

CHP manages 308 community-based health buildings providing around 900,000m2 gross internal area (GIA) of space and the clinical capacity to deliver over 100 million patient appointments per year.  Our buildings are multi-use, providing a range of services including GP practices, diagnostic care, community and mental health, pharmacies, outpatients and local neighbourhood services.
