Community Health Partnerships has published its Forward Strategy, which sets out our direction of travel over the next five years.
The NHS continues to transform at a time of significant pressure and financial constraint. It is more important than ever to get the most value for patients out of every pound of taxpayers’ investment. Existing capital investment needs to support improved population health, tackle inequality of access and outcome, support broader social and economic development, and achieve greater utilisation and increased productivity.
Our focus will be on securing the future of the LIFT portfolio as core assets. We will work with partners and customers to identify the strategic need for the buildings, promoting them as key local cornerstones of place-based care. Maximising their use will be critical, and will be bring significant benefits for staff, patients and local communities.
As a DHSC owned NHS Company CHP works in partnership with local health and care systems. Our purpose is to provide innovative and sustainable spaces for patient care. Our modern buildings are underpinned by their quality, value and adaptability.
Enabling a sustainable NHS
The NHS estate has a crucial role to play in supporting delivery of a sustainable health service and enabling system transformation. With ICS infrastructure strategies in development and most of the LIFT assets designated as core, we have a great opportunity to support the shift of care out of hospital, recovery of elective care, via initiatives such as community diagnostic centres, and the integration of primary care. Thereby demonstrating the value of the LIFT assets at the heart of place-based care and promoting greater patient throughput.

The LIFT buildings are key local cornerstones to deliver place based care. Our strategy is built around these as core assets and optimising their use. We will work with partners and customers as part of the local integrated systems to provide facilities solutions to deliver more care and better health outcomes in local communities. The CHP Five Year Strategy 2023-28 (PDF) sets out how we will do this by supporting ICSs with the development and iteration of their place-based care plans and infrastructure strategies, providing data insight and innovative tools to support decision making and core asset optimisation. We will align our regional multi-disciplinary teams and delivery plans to deliver shared objectives, providing skills and capability, and additional services, where required. In effect, we will be more customer focussed, be clear on our offer to the NHS and we will communicate this to our customers and partners, showcasing what we do well.
Value for money
With constraints on our own finances, we will need to adopt a more commercial approach and review our future operating model to find efficiencies and cost savings. This will mean a shift in the way we operate and the culture that underpins. An internal transformation is already underway to ensure we are fit for the future.
In addition, over the next five years CHP will need to engage on the future options for the LIFT assets as the current lease terms expire. CHP will lead this programme, engaging with key stakeholders and partners on the options and aligning the assets to future need securing them for the long-term benefit of patients and local communities.
Skills and capabilities
This is an ambitious strategy, we have a committed and resilient team at CHP who have strong sense of purpose. We will use the LIFT assets, together with our knowledge, skills and capability to support the NHS deliver place-based care and improve population health outcomes.
