A workshop aimed at highlighting good practice and boosting neighbourhood delivery of clinical and care services and out of hospital services was hosted this week in South East London by Community Health Partnerships (CHP) and South East London Integrated Care Board (ICB), supported by Partners Lexica and NHS Confederation.
The Southwark Workshop exploring ‘The role that our buildings and other community spaces play in the development of Integrated Neighbourhood Teams and the delivery of clinical and care services at a neighbourhood level’, focused on transformative out of hospital shift and integrated neighbourhood team services and their requirements for supporting physical and digital infrastructure. These initiatives align with the Government’s direction of travel towards greater place-based delivery.
NHS Service Leads and Estates professionals from across Southwark’s GP community, Community and Trust Providers, voluntary sector and partners Lexica, NHS Confed, NHS Property Services, and City Health Care Partnership who showcased the Jean Bishop Case Study in Hull, attended to share experiences, learning and approaches to how best to enable clinical strategy delivery in line with the developing models of care and the expected NHS 10-year plan.
The event was chaired by Dr Nancy Kuchemann, GP and Co-chair of Partnership Southwark along side Deputy Medical Director of the South East London Integrated Care. The event was followed by a tour of the Tessa Jowell Health Centre in Dulwich, a NHS LIFT building which integrates GP services, renal dialysis unit, diagnostic testing, as well as mental health support and an on-site pharmacy all under the same roof.
The success of the Jean Bishop Integrated Care Centre in Hull and how its frailty pathway has brought services together in the local community to support older people was a highlight; and Confed Senior Policy and Delivery Manager (Primary Care) George Johnston set the scene and the context for the importance of local delivery, drawing upon some good practice examples. Lexica delivered a presentation on recent work they have delivered for Guy’s and St. Thomas’ Hospital looking deep into clinical vision and requirements for services and identifying which services could relocate out of the acute.
Local successes, including the recently opened Community Diagnostic Centre at Eltham Community Hospital, were highlighted to demonstrate what can be achieved when partners work together to benefit patients and clinicians. The Eltham CDC will provide up to 94,000 additional scans per year, alongside a range of other community health and care services.
The participants discussed how to overcome barriers to delivery across organisations and services, and how accurate data and modelling can contribute to effective planning and delivery of services where they are needed most, making best use of existing estate.
Using Tessa Jowell Health Centre as a neighbourhood-focused building example, service leads were able to identify current barriers to enabling an Integrated Neighbourhood Team function.
Following the session, key opportunities to support these agendas will be identified from the break-out session outputs, to ensure real actions are taken forward.
