On Thursday 3 April, the SWAGGA team partnered Pumping Marvellous to deliver a heart health check event at CHP’s South Liverpool Treatment Centre.
Over the course of the morning over 740 heart checks were conducted, including ECG, blood pressure, cholesterol, and glucose tests. This type of event supports the Government’s key mission of prevention over treatment and on the day checks resulted in 4 people being referred for follow up of potential heart failure.
The event also highlighted other community services in the local area. The Living Well Bus was on-site, offering convenient COVID-19 vaccinations for over 75s, and inside the centre the SWAGGA team provided information on Ageing Well, Cancer Screening, Mental Health and Learning Disability support. The SWAGGA Care Coordinators were readily available to provide essential information on screening programs, signposting to relevant services, and book appointments for those in need.
Also on the day, the team conducted 37 frailty reviews directly reducing the need for these patients to visit their GP – a testament to the efficiency and effectiveness of this community-based approach, which CHP supports.
The SWAGGA team were joined by Pharmacy students from Liverpool John Moores University (LJMU) who played a vital role in supporting this very busy event. This provided the students with invaluable hands-on experience in patient care and community health initiatives.
Who are SWAGGA?
The SWAGGA PCN Network is made up of 16 GP practices in South Liverpool covering Speke, Woolton, Aigburth, Garston, Gateacre and Allerton.
Primary care networks are designed to provide proactive, coordinated care to their local populations, in different ways to match different people’s needs, with a strong focus on prevention and personalised care. This means supporting patients to make informed decisions about their own health and care and connecting them to a wide range of statutory and voluntary services to ensure they can access the care they need first time. This event is just one of many ways SWAGGA proactively support patients in the local community.
