Community Health Partnerships (CHP) has published Transforming Neighbourhood Health, a report into how its place-based estate, developed via LIFT Public Private Partnerships, is already enabling the delivery of neighbourhood care in local communities.

This is providing better environments for NHS staff to deliver excellent quality care, resulting in improved patient outcomes and experiences.   

“To deliver Neighbourhood Health Centres in every community the 10 Year Health Plan sets out the need to make better use of the existing estate.  We have shown through strategic capital investments how to adapt and repurpose space we can drive productivity and value from existing community-based assets, supporting the shift of care closer to home and the implementation of neighbourhood care.  The impact of our work speaks for itself, I hope it inspires our partners to act on future opportunities.”  

Transforming Neighbourhood Health highlights real examples where CHP has enabled neighbourhood health services and care closer to home.  These include: 

• East Birmingham Locality Hub at Washwood Heath Health and Wellbeing Centre, referenced in the 10 Year Health Plan (page 33) has seen A&E attendances fall by over 30% in the last 12 months among targeted patients, and length of hospital stay fall 14%. 

• The opening of a Community Diagnostics Centre (CDC) at Eltham Community Hospital, South East London provides 94,000 additional diagnostic appointments per year, including 10,000 MRI and CT scans and 28,000 phlebotomy tests. 

• Over 500,000 additional diagnostic tests, checks and scans delivered through investment in Community Diagnostic Centres (CDC) and services since 2021. 

CHP manages 308 community-based health buildings providing around 900,000m² gross internal area (GIA) of space and the clinical capacity to deliver over 100 million patient appointments per year.  Our buildings are multi-use, home to a mix of tenants and services including GP practices, hospital trusts, community health providers, mental health trusts, pharmacies, diagnostic services, outpatients and more.
