Estates Strategy and Optimisation

We support the NHS to deliver the Long Term Plan and its focus on out of hospital care by providing high quality infrastructure and services that enable a fundamental shift in care delivery into primary care and community settings.

The Fuller Stocktake Report (2022) report reaffirms the vision to integrate primary care into communities and the estate is a key enabler to delivering this model of care.

Strategic Partners focused on population health outcomes through optimised infrastructure

Our people understand healthcare delivery and are experts in the provision and management of primary, community and mental health infrastructure. We work with partners in an advisory and delivery capacity at national, regional and local/place levels. We enable the NHS to deliver services that meet population health need while optimising the estate to obtain maximum value from the out of hospital community-based infrastructure.

Our people actively participate in local conversations in order to align the LIFT portfolio and the wider NHS estate to support emerging clinical service models and service contract needs and the delivery of whole system estate strategies.

ICS Infrastructure Strategy support

Our Strategic Business Development team supports NHS partners with strategy development, driving estate optimisation working closely with our Property and Operations teams, LIFT partners and wider NHS estates colleagues including Trusts, PCNs and NHSPS.

Our Regional Directors are well placed to mobilise quickly to support Integrated Care Systems to develop and implement their ICS Infrastructure Strategies bringing local knowledge and key stakeholders to take forward discussions and delivery. Our advisory services include health service and estate planning, data analysis and insight, technical property skills, access to established supply chains and acting as the informed client.

Strategic Delivery Programmes

Our Strategic Business Development Team works closely with NHS England’s National Estates Team who commission us to lead on initiatives that support strategic estates planning and optimisation. We provide programme management and delivery capability that brings significant benefits for the NHS.

Two key programmes we have delivered recently have been the Primary Care Data Gathering Programme and the Primary Care Network Services and Estates Planning toolkit.

Primary Care Data Gathering Programme

The Primary Care Data Gathering Programme is a national initiative delivered by CHP on behalf of NHS England. The programme has established a consistent baseline of data for every NHS reimbursed GP practice across England, with outputs visually displayed and accessible to STPs/ICSs, CCGs and PCNs via SHAPE Place Atlas.

Primary Care Network Service and Estate Planning

Community Health Partnerships worked with the National Association of Primary Care (NAPC) on behalf of NHS England, to produce a Primary Care Network (PCN) Estates Toolkit to provide PCNs with a flexible framework and support process for producing robust primary care investment plans with clear priorities that align to wider ICS strategies.
