Primary care data gathering programme

The Primary Care Data Gathering Programme is a national initiative delivered by CHP on behalf of NHS England. The programme has established a consistent baseline of data for every NHS reimbursed GP practice across England, with outputs visually displayed and accessible to STPs/ICSs, CCGs and PCNs via SHAPE (Strategic Health Asset Planning and Evaluation) Place Atlas (shapeatlas.net).

Outputs will inform future use of capital investment as a critical part of system planning, with a focus on improving safety, transforming the patient pathway and working environment, with resulting benefit of reducing future revenue operating costs

All data collated as part of this programme will be published on SHAPE, the web enabled application that links national public health, primary care and demographic data with information on healthcare estates performance and facilities location. Data will not be publicly available and only accessible to and between those having relevant permissions enabled, with this requiring Data Sharing Agreements being entered into in some cases.

CHP, supported by its LIFT partners, has worked with all 42 Sustainability and Transformation Partnerships (STPs), Integrated Care Systems (ICSs) and CCGs to collate any existing, relevant estates and operational data held regionally.

In addition, three facet surveys were conducted on GP practice premises where existing information isn’t held.

The primary care data gathering programme provides national estates data from September 2022 for 8,683 GP premises. It covers patient list size, GP workforce, premises ownership, size of demise, rent, and backlog maintenance data. In addition, it covers site usage type, age of the premises, EPC/DEC ratings and type of heating.

Access to the toolkit is freely available via a simple registration process which enables the download of the generic templates and guidance notes.

This work is pivotal to enable a clearer understanding of the condition of the primary care estate and provide significant data analysis and reporting tools available to the local health economy via the PCDG SHAPE Atlas. This will assist key decision makers including Primary Care Networks to make informed decisions for the future of healthcare within their local premises.
